Forgetting a document or a medication list can make a hospital appointment longer and more stressful. Packing the night before helps you leave calmly and arrive ready.
Use this checklist as a starting point, then add anything your hospital or care team has asked you to bring.
Essential documents
Checklist
- Your provincial or territorial health card, or other coverage information.
- Photo identification.
- Your appointment letter, referral or confirmation email.
- Private insurance details, if you have them.
Health information
Checklist
- A current list of medications, doses and supplements, or the containers themselves.
- A list of allergies.
- Recent test results or records you were asked to bring.
- Your questions for the care team, written down.
- Contact details for your family doctor or pharmacy.
Comfort and practical items
- A charged phone and charger.
- Glasses, hearing aids and spare batteries.
- A water bottle and snack, unless you were told to fast.
- A sweater, as waiting areas can be cool.
- Something to read or listen to during waits.
- Payment for parking.
What to leave at home
Leave valuables, large amounts of cash and unnecessary jewellery at home. If you were asked to fast, avoid food or drinks as instructed. Some areas restrict scented products, so check before wearing perfume.
If someone is coming with you
A companion can help take notes, carry items and remember instructions. Share your questions with them beforehand. If you use a mobility aid or need assistance, bring what you use every day and confirm accessibility arrangements with the hospital in advance.
Frequently asked questions
- Do I need my health card for every hospital visit?
- Hospitals usually ask for your health card at registration. Bring it to every visit unless your hospital tells you otherwise.
- Should I bring my medications?
- Bring an up-to-date list at minimum. Some departments prefer the containers themselves. If unsure, ask when you confirm your appointment.
